Call today at 206.708.1845 and our helpful staff will walk you through setting up an appointment. To expedite your office visit, please download and bring your completed New Patient Information forms with you. If you'd like to fill out your registration by hand at our dental office, please arrive 15 minutes prior to your appointment.

Payment options

In an effort to reduce cost to you while maintaining the highest level of professional care, we have established a financial policy as follows:

  • Payment is due at the time of service.
  • We accept Visa, MasterCard, American Express, and Discover.
  • Payment plans are available through our Dental membership plan: www.carecredit.com

Insurance

If you have insurance, we will help you determine the coverage you have available. To best assist you, it is important that you submit your dental and/or medical insurance information to us a minimum of two business days prior to your scheduled appointment. This allows us time to contact your insurance company and obtain a breakdown of the benefits you or your employer purchased from the company. We will be happy to help you understand your policy.

Should you be diagnosed with treatment services, we will be able to provide you with a detailed plan which will give you an estimate of how much your insurance company may pay for each procedure, as well as the portion you will be expected to pay at the time of service.

We are a PPO provider and are contracted with:
Aetna, Ameritas, Assurant/Sunlife, Delta Dental, Guardian, Humana, Principal, and Regence. Information can be found on your insurance website confirming if we are in-network with your plan.
